✓ Medical History GatheringNice program. Very intuitive & highly customizable for an app. I couldn't get the auto submit to email to work via kiosk mode...just a bug that might need fixing. Love the app but it has little use for me until old records can be recalled and edited in the kiosk mode. (We need it for a waiting room to enter medical history. We want patients to update their information without having to re-enter all their data from year to year. Due to Hipaa we have to use the kiosk mode. Tried reaching out to the company to discuss this but their was no contact information that I could fine... Maybe they'll read this review & hook us up 😉). Overall a really cool app though. ✓ Just a reviewMy company is trialing this app for forestry field work. It is a simple-to-use app with a helpful formating screen/background when creating forms; the text-box options are great (e.g. when collecting numerical data, you can format a box to calculate the sum of the data collected). One negative is with text/box-heavy forms, the software bogs down and is very slow (e.g. takes 4-5 seconds to flip from one page to the next), which can be cumbersome in the field..Version: 1.6.0.1
✓ Excellent customer serviceI have been using FormConnect pro for a while to capture client notes and recently encountered a freezing glitch. I contacted support and was blown away by how fast they responded to me, first by email and then followed up with a phone call. Once they understood what was happening they quickly went to work to resolve the issue. I am very impressed since the only other time I have had that fast of a response was when I utilized a program a sister built for me!.Version: v1.8

✓ Love this app!Great little app! I wanted an app that helped me move all my paper forms over to an iPad app which was easy for construction workers to use without specialist training. After making some enquiries with a number of specialist providers and getting quotes in excess of £150 per annual licence, I heard about FormConnect Pro which has been the best “Well, it’s worth a try” decision I’ve made this year! ✓ Great app for commercial real estate appraisersI am using the app to create inspection reports for my team. So far I love the flexibility and ease of customization. Only drawback I see is that ir currently doesn't appear to support multiple device syncing. This feature is a must, as almost every user has more than one device (ie iPhone, iPad, etc)..Version: 1.9

✓ $25k app for $15 bucks? Hell yes!For our new company we needed an app that kept track of multiple items ie time in time out cash in cash out etc. We are in a business that is heavily regulated by multiple government agencies and any error costs us in fines. I searched high and low for a developer to do this for us and every answer came back the same "a minimum of 20-25k for this, but plan on 25k for a barebones app." I spent months searching and somehow ran across Form Connect. I bought the app on my wife's iPad and a few hours later I had my 25k app hammered out. We love this app, LOVE IT. If you are hesitating purchasing it, don't, it works! ✓ So easy!Once you get to understand the form building tools is so easy to produce a form for whatever occasion. In 15 minutes yesterday I put together a form to cover risk assessment to comply with the service users requirements while I was still on the work site. We were awarded the purchase order and started the work because of the speed at which the assessment was was produced. Our competitors were still on their way back to their offices to change their standard risk assets comply. Electronic reporting that can include photo makes life soooo easy and there’s no paperwork to get lost.👌.Version: 2.7.5

✓ Very easy to create my database form!I have been using a database app on my iPad for a few years and have been running into some glitches so I am looking for a change. (I work in the educational setting and am keeping track of student goals and services). I came across FormConnect and took the plunge to pay for the pro version. It was very easy to create the fields for my database and this just may meet my needs. I do have some requests… 1. I find the background very distracting and cluttery. I would like an opportunity to make my own background…. or have plain colors 2. I would like to be able to have my databases in different folders 3. I would like to have more theme choices… or the opportunity to set my own colors. 4. I would like the opportunity to sort/generate a report based on specific fields. 5. With a drop down list, it would be helpful if there is a quicker way of adding another item in the list rather than going thru full screen edit. 6. It would be nice to press one button to alphabetized the drop down list. That’s all I can think of right now. I will update this report as I am working with this some more..Version: 1.7.0.1
✓ Form Connect ProThis is one of the better form apps currently on the market, it is simple to use, extremely stable and provides good results. My use was to quickly compile real estate inspection data for presentation to clients in a standard format, combining photos, text, checkboxes and dropdown lists. If I was to suggest areas of major improvement it would be in exporting as a pdf file - it fails to export the actual screen pages, rather it condenses into a raw format with little appeal for the end user. In my case, to provide a pdf report in an appealing format I had to take Form Connect screenshots to camera roll, edit these screenshots to remove the background edges and compile the png files back to a single pdf file, then compress the new pdf via a third party program and export via dropbox. This is a lot of work to get a multi page report into a format presentable for distribution - if the pdf export was the same as presented on the screen (WYSIWYG) it would be a giant leap forward and make this app a difficult one to compete with. When a text box is located at the bottom of the iPad page the virtual keyboard presents issues that are quite annoying and some of the text formatting has minor issues when changes are made. It’s still a great program, but the pdf export issue is enough for me to consider looking elsewhere if it is not included in a future upgrade - it may be more suited as a general database entry tool, whereas I require instant glossy reports in png or pdf format for simple electronic distribution as attachments..Version: 1.7.6
✓ Computer Repair Tec.For a long time I have been looking and trying so many form programs only to be disappointed. But not with this App, Formconnect has now made my job a hole lot easer. From setup to finish it was incredible how easy it was. No more long inputting for info on systems coming into shop. Just one click and a few taps and I’m done “:). I to had a question for the support team and 3 mins after posting Steve called me. That never happens. I’ve spent a lot of money just try different apps. Till now. ✓ Excellent, without being over complicated!After reviewing and reading reviews for over a month I came upon FormConnect and was instantly excited about the prospect of capturing the data I needed, without having to pay for things I didn’t need. It did take a bit to get started and when I had a question I submitted a customer service request. I received a call the same day and Steve spent over an hour going through the questions I had, and the functions he felt would be helpful. ✓ Great and easy to use applicationI was looking for an application that I can use to create forms on the ipad. I am used to using adobe CC for my forms buy really needed something on my ipad. You can create forms, use the pro version for field calculation and store them on your ipad. Then you can make signature boxes where signatures can be drawn and then email the completed doc as a pdf, or other file format to a recipient. This application can be improved to make it a 5 star product. But 4 stars is definitely earned so far..Version: 1.6.0.1
