✓ Wasted my time & money -_-I was excited to use this app for my business. I have clients that have to fill out waivers & release forms for services & was tired of printing out the forms. I thought it would be great to be able to have an electronic version that I could send my clients prior to their appointment so they could fill them out & sign them & return back to me. Saving time & paper. The description says "be able to share with others". What it doesn't say is that the "others" must have an iPad & FormConnect on their iPad to be able to complete it. Very disappointed!! Also the customer service response when I asked a few questions was very poor. Basically a one sentence reply with best regards, then the reps name. If you need this app for anything like I did, look elsewhere do not waste your time or money. All my normal forms are in word format & there is not a away to import these to formconnect. You HAVE to start from scratch creating fields, labels, text the entire form has to be designed..Version: 1.5.9
✓ Data CorruptionThe idea behind this app is awesome. I teach lifeguard courses and have just recently started using this app to record information about my students that I need to hang on to for years to come. I love the ability to create your own forms, move things around, change them after you have already made a couple of entries and have it be active in the previous entries as well. I also love the ease of use to get this program up and running. I have however found a glitch that I am extremely unhappy with. I was doing a test run with 15 entries and attempted to export them to a PDF format for safe keeping. When I did the export, it corrupted one of my files. Upon further tests, I was able to determine that whichever entry was currently open on the screen (highlighted if you will) was essentially changed. In my situation I had a TEST entry, It copied the information from the next entry (Andrew) and renamed the whole thing as my final entry (Yvonne). It did this with every type of file format and with any available program. As a business app, the fact that files are being changed during any kind of export means instability to me. It is imperative that I have the ability to copy my files fully, accurately and completely without the concern that the app is going to overwrite something. When this problem gets fixed, I would be happy to provide 5 stars to this app..Version: 1.4

✓ Constant crashingIt had such promise, and i have up upload reguarlly between crashes and that works but have no deleated it when during a meeting when i was preparing a family for their funeral and getting details about their deceased. And during several mins of conversation and taking notes it crashed deleating everything I had just written, it would have been to traumatic for them to relive the conversation. ✓ Need more for $$$Would give 5 stars if it had the ability to be opened in Word. Clients love signing the contract on the iPad, but can't open questionnaires without converting from PDF and not everyone can do that. Would like to be able to have different fonts though so we can better reflect our image. Also wouldn't mind having a scale for our post event questionnaires. A few more backgrounds or the ability to create your own might be a good idea. I'd like to have each of our questionnaires and surveys with a different "cover" that includes our logo..Version: 1.5.6
✓ Good app but could use some additional "user friendly" featuresI like the ability to create forms based on my needs. I upgraded a year ago to formconnect pro. I keep having problems with the app crashing in the middle of use with clients - this is extremely inconvenient and embarassing. PLEASE FIX THIS GLITCH. Customer service could be friendlier - I wish there was an option for providing suggestions. I've emailed suggestions and have received defensive emails in response. I would recommend allowing suggestions or surveying users for needs. I'd like to see the ability to select all and change font size or characteristics for text that has already been entered. I'd also like to be able to either "hide" templates that I don't currently need or arrange templates in a preferred way instead of it automatically going alphabetical. Finally, it would be great if a specific text field could be set to carry over into multiple pages (e.g. customer name) and if pages numbers could be added (e.g. page x of y)..Version: 1.6.0.1
✓ Good, but...This app has great potential, but for me it's currently marred by frequent crashes though, to be fair, I've not suffered any data loss so far. I've also found that the location finder remains live and doesn't hold data if the form is multi-page. Move to another page, then return and your location data has disappeared. Another pet peeve is the manner in which images have to be loaded in to their containers. Forget copy and paste: the only way to insert an image is to first save it to Photos, and then select it from the container popup menu. It's a tiresome process when you're dealing with several images per record. For simple text data collection though, it's definitely up to the task..Version: 1.5.6
✓ DO NOT BUY!This could be good...but it's not. There are simple, obvious things that it doesn't do - like change the order of the folders. The only way it seems you can do that is by erasing the folder - including ALL its content. Also, there's no back button so when you mistakenly brush the screen, things you created move or change, and it can take a while to fix - especially if you're trying to move a group of items. And there's no way to change text colors. It seems like the creators abandoned this and aren't making any improvements. I might try to use it because I payed around $12 for it, but I'll be looking for a replacement. UPDATE: Hahaha! There's just ONE place where you can change text color - and it's one of the LEAST used options. I honestly thought I missed it so I looked and looked, but couldn't find it. Then, after HOURS of trying to work with it, I stumbled across it. It would be great to see the changes you mentioned. A back button would go a long way. Being able to change the background would be a nice touch. And being able to change text color in the other 99% would be great!.Version: 1.7.9

✓ Not bad but needs some improvementThe forms program is not bad. It does what it is intended for. There is a problem on my ipad 2 that displays 3 rectangular fields at the top of every form. the ones program comes with and the ones i make. I believe this to be a software problem and hopefully it will be corrected soon. Its really in a bad place. The cost of the program is probably a bit high for as simplistic as it is at this point. Hopefully an update will give it a bit more robust look. The issue with the boxes has resolved after i did a sync on my computer..Version: 1.01

✓ Could be so much moreGem of a great idea for an app with some nice data collection and export features however... Disappointing implementation because: Can't remove fields once you've created them. Can't rename fields. Can't delete forms (not in this latest update). No calculation fields. Stuck with the 6 or 8 form themes which look ok but could be better, in fact it appears the themes have had more development time than the apps overall functionality. Desktop image is simply awful and needs changing to make it user selectable or a plain background. Icon image for forms could do with the same makeover. If the app design had followed the Apple GUI theme / template / guidelines it could have looked slicker and more professional. it the app gets some of the above changes then it could be so much more and a really cool niche app..Version: 1.4.1
✓ Refuses to test before pushing updatesEvery update they break something and then take two updates to fix. This time they broke exporting data which is the prime function of this app..Version: 2.7.3
✓ Signatures don't transferThere are so many things I love about this app. But one of the most important reasons I purchased this app was so that I could have forms where I could collect and maintain client signatures long term without a paper backup. A couple of weeks after I purchased the app I also purchased a new iPad. When you transfer data from one iPad to another, everything transfers except the signatures. I asked the company for support and at first they responded, but when even the solution they offered didn't work and I asked for another option they stopped responding. It's really unfortunate because this is a pretty good app in all other ways. But not having signatures on consent forms exposes my business. This app desperately needs to be updated so that when users change iPads or have multiple devices they are using to manage forms all data including signatures transfers.Version: 1.7.3

✓ Okay, butForms are easy to create and enter information into, but there is just too little flexibility. You can't design backgrounds, change colors, etc. However, the biggest reason I gave it three stars, is because it doesn't do any type of calculations, so it is pretty much worthless for invoices or any other form that needs to track quantities, determine totals, etc. I wanted to create a form that I can do time studies with, so I would need to enter counts at various times and have a running total for each record. If they add the ability to do calculations, I'll gladly change my rating to five stars..Version: 1.4
✓ Used to be excellent....I've had this for a long while and found it really good. However having just upgraded to the latest version I have two complaints. The record scrolling arrows have been removed and when moving through records the list keeps recentering which makes it less easy to scroll. Secondly, and this is a major degradation, the performance in moving through a record set is painfully slow. I have a database of some 500 single page records, each with a single image and 10 text fields. Using the older version moving from one record to the next was almost immediate. With the latest version it is taking between 5-10 seconds for the next record to be displayed. Coupled with the change to how the record set is moved through, this is a major disappointment. Had I known I would not have upgraded. The App Store images are also misleading as they show the old record layout with the scroll arrows!!!.Version: 1.5.6
✓ Latest update data loss (v1.7)The latest version did a database update on first opening. Now every Page 1 (of 3) of any forms I have created are blank. As this contained client information to identify them Pages 2 and 3 are worthless as all Records are "unnamed". Prior to this really did enjoy utilising this app, but this is ridiculous, especially as all templates that came with the app have been updated unscathed..Version: 1.7
✓ App crashesI previously had this app on my iPad 4 and never had an issue with it. Now with the new iPad Air on the latest OS it will not keep running it just crashes all the time. I checked forums in regards to this and found a large number of very dissatisfied purchasers with the same problem..Version: 1.7.1
✓ No RelationsThere are no relational fields, so you have to manually enter data for the same thing or person in other forms. At this time there is no calculation field. If you want multiple things calculated, you will have to calculate them yourself and put it in a text/numerical box. I like the form layout, but the functionality of this app is not very useful for me. I need an app that will bring data from other tables for multiple forms. If you are wanting this for invoicing, you will have to enter the same customer information again instead of using a master list or relating back to the same customer info from a previous invoice. I don't think $10 is worth this app at this version..Version: 1.4.2

✓ Easy form with great flexibilityI'm running a small IT professional business and have currently utilized iPads for our database. I've spent time and money trying to find a simple and easy solution for our database. Then, I stumbled on Formconnect. It is a great app with a good degree of flexibility and customization for your needs. The form is very easy to work with and it offers a good portability in terms of exporting the information to a database on a different desktop or laptop computer. The support team, lead by Steve Hoffman has been so helpful and offered an unprecedented service. I'm happy to recommend this product to those business owners looking forward a simple solution in their database..Version: 1.4
✓ Looks good on paper...Very disappointed with this app, had high hopes that it would reduce paper forms but it doesn't. It has so many frustrating bugs eg when cloning a record for a new client it deletes all of the general information despite checking the box asking it to keep this information when cloning, completely useless! ✓ Ok but a bit limitedForm connect is a nice interface with clear form creation. As per previous review. Tap forms does offer so much more customisable features, just not so pretty. This form database lacks the ability to link to a contacts list so does require multiple input of the same details, if you create a booking, quotation and invoice for the same customer for instance..Version: 1.3.3

It is a real shame that quality of this pricey app is a big question mark. I do appreciate its design but that is all. ✓ Mostly Great AppI purchased this app to make custom order forms for my home based bakery. I love being able to make customized forms and fill them out, however when I organized them by date, the date shows up year, month, then day, which to me is very confusing to get used to this format! There is no way to change it and when I contacted the makers about it they said there was no way to provide an option to change the format, which I have a very hard time believing. Over all the app will work for what I need it to do but isn't what I hoping for as far as usability after the form is filled out..Version: 1.5.3
✓ Good app, needs a better way to export dataThis app has great potential. The form designer part is really good, making it easly to create your own forms in any layout you want. Filling them in is a snap too, taking full advantage of the iPad user interface. Where the app needs work is on the export side. Right now the only way you can get data out of the app is to email one form at a time, in one format at a time. For a use as a business app, it needs to have a way to export multiple forms in multiple formats (csv and pdf) directly to a server. Until it does this it will remain an app for consumers and won't be much good for most businesses. It also needs a means of prepopulating some fields. It also crashes from time to time. But it's a great start and I look forward to future releases..Version: 1.0
